A Duke Boot is utilized for reduction of edema to promote wound healing. It provides 30 mmHg compression to the lower leg. It can also be utilized for leg edema with or without venous ulcer. Particularly useful when the tissue seems fragile and susceptible to tearing.
If a sound is present a hydrocolloidal dressing is applied followed by layer of viscopaste. Viscopaste is zinc impregnated gauze wrap which helps to keep the skin soft and supple. The viscopaste is then followed by a layer of Coban. Coban is a stretch elastic wrap and is the compression layer. One wrap provides about 30mmHg compression, two wraps provides about 40mmHg compression. The Duke Boot is left in place for one week.
Manufacturers have developed pre-packaged systems which function similar to the Duke Boot. Some of these are Profore Four Layer Bandaging System by Smith&Nephew, and Dyna-Flex Multi-Layer Compression System by Johnson&Johnson, to name a few.
